At Sound Foot and Ankle in most cases, we can begin your treatment the day of your appointment. To maximize your time with us, please print out and complete the forms below and bring them to your first visit, along with:
• Driver’s License or State ID
• Insurance Information
• Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from prior doctor visits
• A list of current medications and any known allergies
